New Membership for Adult Coaches, Chaperones and Volunteers of J.O. Volleyball Teams

New Adult (JO Coach/Chaperone/Volunteer) On-Line Delta Region Membership

Background screening is required every two years.  Here's the paperwork needed after you finish the on-line renewal of your membership.

• If you choose NOT to use the Electronic Signature Option, the on-line system will produce a manual Waiver & Release of Liability you'll need to sign & give to your club director to complete registration.
• • Background Screening Consent Form (required every two years--the on-line system keeps up with this.)
  • Chaperone Responsibility Form (chaperones only)
  • Volunteer Waiver & Release not a coach or chaperone, but work with your junior club?
  • Volunteer Consent Form  not a coach or chaperone, but work with your junior club?
  • Non-Citizen Coach Background Screening Consent Form Green Card/Visa number (no Social Security number)
  • Non-Citizen Coach Procedure

• Note:   Coaches must be at least 18 years old.  Chaperones must be at least 25 years old.

Explanation of optional fees appearing on on-line registration: 

• $5.00 High-Performance donation will be paid directly to the national HP program (not the Delta Region program) to support Youth National Team and A2 & A3 camps.  If you check this option, you are obligated to pay the $5 fee.  Most members would prefer to make donations to Region-based programs
• Extended officials' insurance: $6.85 additional fee is ONLY for PAVO officials (not ANY junior officials)  It is highly unlikely you should include this fee
